How to register

Patients wishing to register with Richmond Clinic will be asked to fill in a Registration form and also a questionnaire.  (Please provide as much information as possible).  Hand the form back into registration when complete.  Any documents relating to proof of identity/address should be emailed to the practice enquiries.richmondclinic@wales.nhs.uk .  You do not need your NHS medical card, but the name and address of your previous doctor and NHS number will speed up the registration process.
